This Ministry of Health form is called the HP3823 form, and is not yet available as a downloadable pdf. If you want it, you have to request it from your doctor.
A scan of the form is attached. You can expect denial that such a form even exists, and you can also be told you are NOT allowed to opt off.
If that happens, and you get nowhere with the NIR themselves please contact the Health and Disability Advocate system (http://www.decisionmaker.co.nz/guide2003/hlw/health.html), who have already had to deal with several complaints about NIR obstructiveness.
They have been well versed as to the rights of parents to opt off.
If you continue to have problems, contact me.

Just wondering,
I have had these people I think turn up on my doorstep after I had a few phonecalls and told them to not call etc. They came with their vaccination kit and wanted to vax him on the spot.
Prob is my hubby doesnt know or understand my choices not to vaccinate but he is a piece of work and I want to make sure these people never contact me again ever.
Is this the form I use?

Yeah they got the information from my GP that I had imms that were due for Tommy. I was in such a flap by the time she left I think I felt sick for a week! I just told her I was doing them through the doctors surgery....I lied but I could not be bothered with the HUGE bleat that she was about to burt out ay me. Im worried that I wil soon get a letter from the doc and another visit form the vax people and I don't want that!

They turned up on your doorstep :eek: Gee these people are crazy! Whatever happened to freedom of choice :confused:
I guess I'm lucky then that my GP has the attitude that as long as I've done my research my decision is just that. My DP is onboard too thank goodness though more in a "yes dear" sense than anything :D

Whats worse is that one of my worksmate's Mum works there! haha she always goes on about how lazy parents not vaccinating their kids etc etc....yeah lazy....right!

I would tell them that you will be taping all future calls where you tell them to leave you alone, and if they continue to come to your house, you will videotape their presence, and audiotape their demands, and be putting all the evidence in the hands of your lawyer, as well as the office of the Health and Disability Commissioner, and they will be seeing you in the medical disciplinary court for persistent, unwanted harrassment and bullying contrary to the law of the country regarding "informed consent".
Yes, that is the form. It is available from your doctor's surgery, but don't expect them to do it without being ultra pissy about it, and possibly getting very snarky at you.
I have a contact at the Health and Disability commissioner, should you need it.

So I've just read over this thread again as it left me a little un easy. Do you think your husband has called the surgery and asked for them to come to your home?

Nope I had a ittle chat to him last night and he thinks (thiank goodness) "immunistaions are sh&^" haha
They called me and at first and said they are out of date please call. So I called back and said please dont contact me I know what I am doing etc etc. So they caled again and I called back again. Then they left a note in my mailbox! Then one day I was home and a knock on the door and she was here to talk to me about overdue imms! I said I have already sorted it with my doctor thanks.She said I can do them now, I said he has tonsilitis and she said oh its ok to immunise a kid with a tempurature and she did it once on a marae and the child didnt have a tempurature 30mins ater because their immune system had kicked in and sorted the probem! She was nuts. According to my workmate that is what her Mum's job is to do, she contacts parents who are behind because they are "lazy". The lady who came to my house said they often visit Daycares etc and vaccinate kids there if they call the parents whie they are there and get consent from the parents because people get behind on imms.
I was seriously mortified. I had a mega panic attack when this ady turned up I was shaking and so upset. I can't believe its illega. If only I knew I would have set my dogs on her haha.Not really but it was such an invasion of privacy.

I think she was from the National Immunistaion Board or something. I will check with the girl at work whose Mum works there on Monday :)
When I posted about it on another forum a couple of people had it happen to them too. Its because I started the imms then stopped when I acquired half a brain! So they thought I had just done it because I couldn't be bothered. I now live in fear they will turn up to his kindy.

I had someone turn up on my doorstep once because K wasnt vaxed against menz b. Luckily for them I wasnt home and they just left a note.
Its full on stuff and its intimidating.

One answer is to always film these people who come onto your property and threaten to show it on youtube and popular web sites. Its your property and if they not happy to be recorded then they must leave.

It's not illegal if they have permission.
But it is illegal if you have specified for them to leave you alone....

My husband and I would have so much fun dealing with them.... :giggle::giggle::giggle:
But it must be not just illegal, but also not allowed (hehe) here in Australia, the most they ever did was send a random reminder in the mail, and back off at the first phone call to ACIR with a request to dare not ever contact me again.

But they absolutely wouldn't take me or you on. We are "known"....
They aim for Mrs Ewen'Mee, or Ms Smith from normalsville.
